Copies of your parents' 2011 W2 wage statements and/or copies of your parents' (and stepparents', if any) 2011 Form 1099 Income Statements. The 2011 Form 1099 statement details income for contract earnings, retirement, pension, annuity, Social Security income (needed for all household members), unemployment benefits, or disability benefits. Please do not send any Form 1099 forms related to 2011 interest or divident income. Submit via mail with this cover sheet. |

Copies of your parents' year-end, final 2011 paycheck stubs (sometimes called "pay advice" or "pay statement" by employers). Copies of your mother's and father's (and stepparents', if any) year-end, final 2011 paycheck stubs are needed. This document will detail all taxed and untaxed compensation as well as all deductions for the full 2011 year. It may be a photocopy of an actual paycheck stub or a copy of an online full year 2011 summary from an employer's website. (This is not the same as the W2 wage statement - both documents are required to complete your aid application). Submit via mail with this cover sheet. |

Noncustodial Parent Profile Form (and 2011 income and tax documents), if your parents do not live together (whether because they were never married or are separated or divorced), the parent (and stepparent, if any) with whom you do not live with must complete the "Noncustodial PROFILE" form. Instructions for the login ID and password will be emailed to you when you complete the CSS/Financial Aid PROFILE. In addition to the "Noncustodial PROFILE" form, your noncustodial parent (and stepparent, if any) must submit copies of the above mentioned 2011 tax and income documents.
