QuestBridge Assumption of Risk, Waiver & Release Form

1. Voluntary Participation. I acknowledge that I am participating in a Quest Scholars Program, Inc. (dba QuestBridge) affiliated Event (each an “Event”) and any activities involved with the Event, completely voluntarily.  I am entering into this instrument in consideration of the opportunity to participate in any such Events, I acknowledge that my statements herein are being accepted and relied upon by QuestBridge and its associates, and I freely agree to the following:  

2. Assumption of Risk and Release. I hereby voluntarily forever and irrevocably assume all risks relating to my participation in the Event, whether occurring prior to, during, or after the Event, and I hereby release and hold harmless QuestBridge, its representatives, officers, directors, employees, agents, volunteers, attorneys, host facilities, and all individuals supervising or assisting with the Event in any way (the “Released Parties”), from liability for any injury, death, loss or damage to property, including without limitation for exposure to COVID-19 and any other infectious diseases, and from any and all claims, causes of action, damages, losses, costs, expenses, and liabilities of every kind or nature arising out of or in any way connected with my participation in the Event or traveling to or from the Event (together “Claims”), and I further agree to hold each of the Released Parties harmless from and against any such injuries, damages, losses, costs and Claims, resulting from any cause whatsoever at any time while attending or preparing to attend the Event, including without limitation for travel to and from the Event, activities during the Event, and lost or stolen belongings. 

3. COVID-19 Waiver.  I acknowledge that QuestBridge strongly recommends all participants of the Event be fully vaccinated against COVID-19 or, if not vaccinated, participate only upon receiving negative results from a PCR test or a self-administered rapid test. 

4. Code of Conduct. I agree to observe the following code of conduct: a) not to consume any drugs or alcohol that will impair my judgment and/or ability to participate in this Event; b) to respect the rights of every individual at the Event; c) to respect all property, including but not limited to the host facilities and equipment; and d) to act professionally.

5. Miscellaneous. I agree that this instrument shall be binding upon my relatives, personal representatives, heirs, beneficiaries, next of kin, and assigns and shall inure to the benefit of QuestBridge. If any provision of this instrument is held to be invalid or unenforceable, this instrument shall be construed as if such provision was not included.  Because QuestBridge’s headquarters are in Santa Clara County, California, and in order to provide certainty in the law, this instrument shall be governed, construed and enforced in accordance with the laws of the state of California excluding its conflicts of law rules, in the state and federal courts in Santa Clara County.
